Recent surveys across Asia and Africa revealed a shocking truth: about 50% of people with potentially infectious TB in communities don't report any symptoms. These individuals can remain in this silent state for months or even years, continuing to spread the disease without knowing it.

The simple active-versus-latent model worked well when we focused on treating symptomatic patients. But to eliminate TB by 2030, we need a more nuanced understanding. The current system can't identify those at highest risk of progression, leading to over-treatment of some and missed opportunities for others.

The International Consensus for Early TB framework recognizes that tuberculosis exists on a spectrum. Instead of just active and latent, it defines multiple states based on symptoms, infectiousness, and disease progression. This allows for more targeted interventions and better patient care.

The new framework distinguishes between infection and disease by the presence of pathological changes. It defines subclinical states where disease is present but symptoms are absent or minimal, and clinical states with clear symptoms. Each state is further classified by potential infectiousness, helping guide public health responses.

This framework emerged from a rigorous consensus process involving 64 experts from around the world. They represented diverse disciplines including researchers, clinicians, policymakers, and TB survivors. Through surveys and meetings, they agreed on definitions that will guide future research and care.

The ICE-TB framework provides clear direction for developing new diagnostic tests that can identify people in different disease states. It helps researchers design targeted interventions for each state and develop better predictive tools to identify who needs treatment most urgently.

The ICE-TB framework represents a paradigm shift in how we understand tuberculosis. By moving beyond the simple active-latent division, we can develop more precise diagnostics, targeted treatments, and effective prevention strategies. This is our path to achieving TB elimination by 2030.
